Medication Therapy Management
Patients eligible for the Medication Therapy Management (MTM) program receive one-on-one time with the pharmacist to review all of their medications and answer any questions they may have. The primary goal of the program is to help educate the patient about their health conditions and medication usage so that they may receive the best benefit from their medications. As a secondary goal, the pharmacist will identify any cost-saving opportunities. Eligibility for the program is predetermined by each patient's insurance, including certain Medicare Part D and Medicaid plans. To determine if you are eligible ask one of our pharmacists or contact your insurance provider. Sessions are by appointment only.
